When you give it to us directly
When you give permission from third parties to share it with us. For example, a family member who has passed on the information with your consent.
We collect personal information:
Name, DOB, Tel number, Email Address, Bank Details as well as information relating to your health and/or personal circumstances.
Staff/Volunteers - we only ask for details that will help us to process your application. Where appropriate this may also include details of any criminal convictions.
People whose data we receive from staff/volunteers such as referees and emergency contacts-
Referees - all we need to know is confirmation that you know the candidate so that they can secure the job/volunteer role.
Emergency contacts - These details will allow us to contact somebody in an emergency.
We may use your information to:
Give you the support service you ask for
To gain a full understanding of your situation so we can provide a personalised service
For internal administrative records (our records or for accounting)
Let you know about changes to our services or about events that might be coming up
Claim Gift Aid on any donations
Ensure we know how you prefer to be contacted
Understand how we can improve our services, products or information
Keep a record of your relationship with us
We may need to share your information with:
Other health professionals regarding your health or care. We will agree this with you. We will not share information unless; you ask us to, we ask and you give your permission, or we have good reason to believe not sharing the information would put you at risk. We do not sell share personal details to third parties for the purposes of marketing.
Other people within Evergreen Care Bexley may also need to access your information to check the quality of our service.
Sharing your story
Some people choose to tell us about their experiences of the health care sector and / or the support they have received from Evergreen Care UK. They may take on a role as an Ambassador or Media Volunteer and attend our events.
This may include them sharing sensitive information related to their health and family life.
If we have the explicit and informed consent of the individuals or guardian, this information may be made public by us at events, in materials promoting our campaigning and fundraising work, or in documents such as our annual report.
How do we keep your data safe and who has access
We ensure that there are appropriate technical controls in place to protect your personal details. For example our network is protected and routinely monitored.
We undertake regular reviews of who has access to information that we hold to ensure that your information is only accessible by appropriately trained staff and volunteers.
We will only ever share your data in other circumstances if we have your explicit and informed consent.

We may periodically change our Privacy Policy from time to time. If we make any significant changes in the way we treat your personal information we will make this clear on the Evergreen Care UK Website or by contacting you directly.

Evergreen Care UK is an award-winning local charity that cares about its clients and community.
Seeing the desperate needs that are, sadly, very often not being met, we believe there is a better way to treat people as they grow older and become more vulnerable.
